Passport forms Form PP1(48KB) to be completed for the issue of new passports, or replacement of expired passports after 10 years from the date of issue or lost or defaced passports
Form PP2(42KB) to be completed for issue of passports to children aged 0-16.
Form PP3(32KB) to be completed for the renewal of passports whose validity has expired after 5 years from the date of issue (in circulation before 1 January 2007), or passports issued with a restriction clause.
